Medicare Claims Guide
If you have an active Chronic Disease Management (CDM) plan from your GP, you may be eligible to claim a Medicare rebate on your physiotherapy sessions. Here's how to submit your claim online β step by step.
A few key things to know before you start your claim.
Make sure you have these ready before submitting your claim.
Follow these steps to submit your physiotherapy invoice through the Medicare website.
Visit my.gov.au in your web browser and log in with your myGov credentials.
From your myGov dashboard, select Medicare from your linked services. If Medicare isn't linked yet, you'll need to link it first using your Medicare card details.
Once inside the Medicare portal, find and click the "Make a Claim" or "Submit a Claim" button.
Fill in the required fields using the information from your Physio Now invoice:
You'll be prompted to upload a digital copy of your receipt or invoice. Use the PDF or image file emailed to you by Physio Now after your session.
Double-check all the details you've entered β provider number, date, item number, and amount β then submit your claim.
After submitting, you'll receive a confirmation screen with a claim reference number. Save or screenshot this for your records.
Prefer not to use the website? You can also submit your claim using these methods.
Download the Express Plus Medicare app from the App Store or Google Play. Log in and follow the same claim process directly from your phone.
Visit any Medicare Service Centre with your invoice and Medicare card. A staff member can help you submit the claim on the spot.
A few details that may help if you run into any issues.
You can update your bank account details through your myGov account. Log in, navigate to Medicare, and look for the banking or payment settings section. Once your account is registered, future rebates will be deposited automatically.
The most common reasons for a rejected claim are an incorrect item number, the provider not being listed on your CDM plan, or an expired referral. Double-check that the item number is 10960, that Physio Now is nominated in your plan, and that your CDM plan is still active for the current calendar year.

CDM plans are typically reviewed annually by your GP. If your plan has lapsed or you've used all 5 sessions for the calendar year, you'll need to revisit your GP to have it renewed or updated for the new year. Your GP can re-nominate Physio Now as part of the updated plan.
The standard Medicare item number for physiotherapy services under a CDM plan is 10960. This is the number you'll enter when submitting your online claim. It's also printed on your Physio Now invoice for easy reference.
The Medicare rebate amount is set by the government and may not cover the full cost of your session. The difference between our session fee and the Medicare rebate is an out-of-pocket cost.
